The mission of Sandhills Medical Foundation, Inc. is to be responsive to community healthcare needs by providing quality, comprehensive, and cost effective healthcare.
Sandhills Medical Foundation, Inc. has been providing quality comprehensive health care since 1977. It is a Federally qualified community health center (FQHC) serving residents of Chesterfield, Kershaw, Lancaster, and Sumter Counties. Community Health Centers are community-based and patient-directed organizations that serve populations with limited access to health care. These include low income populations, the uninsured, those with limited English proficiency, migrant and seasonal farmworkers, individuals and families experiencing homelessness, and those living in public housing.
As a patient centered medical home provider, Sandhills Medical Foundation provides comprehensive healthcare to all individuals. We provide services such as primary care, behavioral health care, onsite pharmacy and lab services, to meet the needs of our patients. Sandhills also provides healthcare services in the community such as patient education, flu shots, and health screenings such as blood pressure, diabetes, and cholesterol and HIV.

Sandhills Medical Foundation, Inc. is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employer under 42 U.S.C. 233(g)-(n). We are an FTCA deemed facility. Malpractice coverage is provided under the Federal Tort Claims Act (FTCA) pursuant to 42 U. S. C. 233(g)-(n).
